Applications are available during school hours at all centers. Call 761- 2023 for information on returning your application. The following documents are required at the time of application:
- Current Tax Return
- Parent photo identification
- Child's Birth Certificate
- Child's Social Security Card
- Child's Immunization Card
- Proof of Medical Insurance
